Welcome to our online bill pay form. Please enter your patient id number from your statement below and click on the pay now button below. Additional information such as payment amount will be entered on PayPal’s secure website.

Online Bill Pay Instructions:

  • Enter patient id (located on your statement) and click on the Pay Now button
  • Enter payment amount in the ‘Item Price’ field in the left column
  • PayPal Members: Enter PayPal login information and complete the payment process
    Non-PayPal Members: Click ’Pay with a debit or credit card’ and fill out the form to complete the payment process
